Those with COVID-19 will be able to access high quality care without leaving the comfort of their own home.
St. Joseph's Home Care is teaming up with the St. Joseph's Health System and Niagara Health in order to launch the COVID Care @ Home Program.
Patients will be able to contact the clinical team over the 24/7 phone line
Those who are at risk of being hospitalized will have access to at home and virtual visits through video chat.
The program will also be able to connect patients with social supports.
It's for patients with a confirmed COVID-19 diagnosis, whether they're living at home, a retirement home or a shelter.
It's more so geared towards older patients who are at risk of a physical or mental health decline.
Those who are eligible for the program will be enrolled at the time of their diagnosis.
