Niagara Health is connecting patients with doctors once they leave hospital, who didn't have a doctor
A new system has been developed at Niagara Health to ensure patients leaving a hospital, who don't have a family doctor, are connected with support once they leave.
The organization says it was started in response to many pediatric patients leaving hospital, but without support for their long-term needs.
It means children up to 18, without a family doctor, will be given a list of doctors and health centres in Niagara currently accepting patients.
Niagara Health adds this will ensure timely vaccinations, and develop monitoring and early detection of health concerns, all reducing the need for unnecessary hospital visits.
It also plans to start this for adults.
