The data covers all levels and age groups, and includes verbal taunts, insults and intimidation.
Officials called over 500 penalties for discrimination, while 415 allegations were investigated after the fact.
This the first ever report the organization has done in regard to the issue.
The report follows a plan released by Hockey Canada in July, as it deals with alleged sexual assault involving members of the 2003 and 2018 world junior teams.
